Merge and reorganize kernel/** in cpp branch

Merge any changes from master for kernel/** into cpp. Reorganize kernel/** into bootstrap, common, delta, platform.

This task will be deferred until after the C++ VM can completely load core libs and the cpp branch is merged into master.
