Cleanup Hash Ruby code

The existing Ruby Hash code is still a mixture of Ruby and primitives. Rewrite Hash as a pure Ruby class. (See also #415.)

Hash has been removed from the C++ VM. The Ruby Hash will be updated once the VM is loading the core libs. This item is now in tracker so closing this ticket.
